There may be a plugin that's conflicting with the gallery. Or maybe something in your theme is conflicting. Have you tested doing it without plugins activated or on a different theme? On the initial creation everything is perfect, but 90% of the time if I go back in and edit the gallery the changes aren't kept. This applies to both adding, removing, and rearranging images. Your other problem shouldn't be happening, however. You might try closing out of your site and going back in. (Maybe your browser too.) You might also try deactivating all of your plugins to see if something is conflicting.
